All student athletes at Guntersville Middle School must have updated records each year in order to participate in sports at GMS. The list of required documents are listed and linked below. 

• Physical Form (completed by student, guardian AND physician)
• Concussion Form (completed by student and guardian) 
• GCS and AHSAA Consent of Participation Form (includes insurance information (completed by student AND guardian)
